A GUIDE FOR SECONDARY LEVEL SCHOOL HEALTH SERVICES

These are the health services that are available to your child while attending Middle School.

1. Mandated Physical Examinations are required for students in grades six and eleven. It is recommended that you see your family doctor for this exam. A private physician’s form is available from the school nurse. A school exam can be done by the school physician or nurse practitioner with written permission from a parent or guardian. You may be present if you desire. You will be notified if problems are found.
 
 2. Sports Examinations are required for seventh through twelfth graders who have signed up for interscholastic sports. A student needs a full physical (done by the student’s health care provider or at school) prior to the initial sport season. After that a re-evaluation may be necessary if the student had a significant illness or injury. The health care provider must sign “cleared” on a PIAA sports examination form.
 
 3. Dental Exams are required for students in seventh grade. It is recommended that you have this exam done by your family dentist. A school dental screening may also be done by the school dentist with parental permission. A private dentist’s form is available from the school nurse.
 
 4. Height and Weight is measured every year along with Body Mass Index (BMI) and a report is sent home.
 
 5. Vision Screening is done every year. Any indication of vision difficulty will be reported to the parent/guardian as soon as possible.
 
 6. Hearing Screening is done for students in seventh and eleventh grades. Parents will be notified if the student is in need of medical evaluation.
 
 7. Immunizations should be brought up to date. For most diseases, periodic re-immunizations (boosters) are necessary to maintain optimum levels of protection. A booster should be received ten years after “baby shots”. The Hepatitis B series, and the Varivax (for Chickenpox) are required for students entering seventh grade.
 
 8. Medication All prescription medication to be taken at school must be in a prescription bottle and accompanied by a signed letter of permission from the parent/guardian. This letter should have the amount of medicine to be taken, times the medicine is to be taken, condition for which the medicine is given, and possible side effects of the medicine. Over the counter medication must be in the original container, accompanied by the same type of note as well.

 

Important things to remember . . .

 

1. Emergency Cards:    

Please complete and return student emergency cards to school as soon as possible. The information on these cards is vital to being able to reach you in an emergency. It is very important that any change of guardianship, address, home phone number, work phone number or cell phone numbers or need to be sent to the school office. Change in your child's health condition should be sent to the school nurse.

 

2. Immunization Requirements:

The following immunizations are required as a condition of attendance at school inn the state of Pennsylvania. Children ENTERING SCHOOL (at K/ grade1) need the following immunizations:

    4 doses of DTaP ( diphtheria, tetanus and pertussis) – one dose after the child’s 4th 

       birthday.

    3 doses of polio

    3 doses of hepatitis B

    2 doses of MMR (measles, mumps and rubella)- 1st dose needs to be after the first

       birthday.

    1 dose of varicella (chicken pox)- dose needs to be after the first birthday

 

These regulations were amended in 1997 and pertain to all students entering school for the first time. Whenever your child receives an immunization you should send documentation to the school nurse so your child’s health record can be updated.

 

3. Physical Examinations:

Physical examinations are required of students entering Kindergarten/ first grade, sixth grade and eleventh grade, and students with incomplete health records. We recommend that you have the physical exam done by your family health care provider, since he/she knows the student and your child's health history. The Coatesville Area School District can provide a physical exam at no cost (with written consent prior to the examination). See link below for a copy of the Private Physician Form

 

4. Dental Examinations:

Dental examinations are required of students entering Kindergarten/ first grade, third grade, seventh grade, and students with incomplete health records. We recommend that you have the dental exam done by your family dentist, since he/she know the student and your child's dental history. The Coatesville Area School District can provide a dental exam at no cost (with written consent prior to the examination). See link below for a copy of the  Private Dental Form

 

5. Medications in School  

It is recommended that No medications be given during school hours. Most medications can be given before and/or after school. If any medication MUST be given during the school day, it MUST be in its original container accompanied by a signed medication form from a parent or guardian AND a signed medication form from the child's doctor (or a signed prescription).

8.      SIGNATURE of PARENT/GUARDIAN

ALL MEDICATION WILL ONLY BE GIVEN ACCORDING TO THE DIRECTIONS ON THE CONTAINER OR PRESCRIPTION

The above information applies to both prescription and over-the-counter medications (non-prescription medications). All medications are kept locked in the nurse’s office unless your physician feels it medically necessary for your child to have the medication with them at all times and the proper documentation from your physician needs to be provided to the school nurse. (The school nurse must also observe that the student uses the medication correctly and responsibly.)  Please see link below.

Kids health is an excellent site sponsored by the Nemours Foundation for both parents and adolescents to research basic health information.  There is information designed for both groups and is easy to access and search.  This is a good place to help with head lice problems.

This is the website for the Commonwealth of Pennsylvania and contains valuable information about health related issues (from A to Z) along with access to statistics, resources, and publications.  One can also order official documents like birth certificates.

www.chipcoverspakids.org

CHIP (Children’s Health Insurance Program) now covers all of PA’s children with free or low cost health insurance.  Parents can get information, phone numbers, and even apply on line.

www.cdc.gov

The Centers for Disease Control is a US government agency and their web site is chock full of all kinds of information.  The site is enormous but is “searchable” if you take your time.  There are sites for kids linked to this site.  Those are listed under the “resources for students” on this LEAP site.

www.aanma.org

The Allergy and Asthma Network/Mothers of Asthmatics site is a great resource for information about:  treatment, medicines, and tips for helping children both at home and in school.

www.nasn.org

The National Association of School Nurses web site has information helpful to both nurses and parents.  There is a long list of web site links that provide resources for parents as well as nurses.
